질병관리청, 지역주민 맞춤형 건강정책을 수립할 건강조사 16일부터 전국 실시[에스피플뉴스] 질병관리청은 17개 광역자치단체와 함께 5월 16일부터 7월 31일까지 전국 만 19세 이상 성인 약 23만 명을 대상으로 '2024년 지역사회건강조사'를 실시한다고 밝혔다. 지역사회건강조사는 '지역보건법' 제4조에 따라 매년 전국 258개 보건소가 34개 지역 대표대학에 위탁하여, 지역주민의 건강실태를 파악하고 지역보건의료계획의 기초자료로 활용하기 위해 지난 ’08년부터 실시해 왔다. 조사방식은 보건소 소속의 전문 조사원이 표본추출로 선정된 조사가구를 방문하여, 태블릿 PC 내 전자조사표를 이용하여 조사대상자와 1대1 면접조사로 진행한다. 조사는 흡연, 음주, 식생활 등의 건강행태와 삶의 질 등 지역별 170~210여 개의 설문 문항을 조사하며, 올해는 그동안 코로나19로 중단됐던 지역별 선택 문항 조사를 실시하여 각 지역에 꼭 필요한 맞춤형 건강정책 수립 및 보건사업에 활용된다. 조사수행 절차는 ➀통계적 방법론에 의한 조사대상 가구 선정, ➁선정된 조사대상 가구에 안내서 우편발송, ➂조사원 가구방문, ➃태블릿PC를 이용한 면접조사, ➄답례품 증정, ➅면접조사 완료자 중 일부(약 10%)에게 설문조사내용 확인 전화의 순서로 진행된다. 조사원은 관할 지역 보건소장 직인이 찍힌 조사원증을 패용하고 있으며, 다음과 같은 복장으로 조사대상 가구를 방문한다. 질병관리청 지영미 청장은 “지역사회건강조사를 통해 수집된 모든 자료는 우리 지역의 맞춤형 건강정책을 수립하는데 활용되는 중요한 자료이며, 조사자료에 대해서는 통계법에 의해 철저히 비밀이 보장되므로 지역주민들께서는 조사원이 방문했을 때 조사에 적극 참여해 주실 것”을 당부했다. 또한, “매년 발표되는 지역사회건강조사 결과를 적극 활용하여 지역 맞춤형 보건사업을 수립함으로써 지역 고유의 건강문제와 지역 간 건강격차 해소를 위해 중앙 및 지방정부가 함께 노력하겠다”고 덧붙였다. 2024년 지역사회건강조사 주요 결과는 12월 발표하며, 지자체에서 2025년 사업계획수립 시 바로 활용할 수 있다.

보건복지부, 박민수 부본부장 주재 「중앙사고수습본부」 제44차 회의 개최[에스피플뉴스] 5월 14일 9시 박민수 부본부장 주재로 「의사 집단행동 중앙사고수습본부」(본부장: 보건복지부장관) 제44차 회의를 개최하여 ▴비상진료체계 운영현황 ▴의사 집단행동 현황 ▴의대증원 추진상황 등을 점검했다. ➊ 비상진료체계 운영현황 5월 13일 평균 입원환자는 상급종합병원 22,271명으로 전주 평균 대비 4.8% 감소했고, 평시인 2월 첫주의 67% 수준이다. 상급종합병원 포함 전체 종합병원은 전주 대비 2.5% 감소한 86,735명으로 평시 대비 90% 수준이다. 중환자실 입원환자는 상급종합병원 2,848명으로 전주와 거의 동일하고 평시의 86% 수준이며, 전체 종합병원은 7,129명으로 전주 대비 2.1% 증가, 평시 대비 97% 수준이다. 응급실은 전체 408개소 중 393개소(96%)가 병상 축소 없이 운영됐고 27개 중증응급질환 중 일부 질환에 대해 진료제한 메시지를 표출한 권역응급의료센터는 17개소다. 5월 7일 응급의료센터 내원환자 중 ▴중증·응급환자는 전주 대비 0.3% 감소, ▴중등증 환자는 0.1% 감소, ▴경증 환자는 3.5% 감소했다. 이는 평시인 2월 1주차 대비 ▴중증·응급환자는 9.7% 감소, ▴중등증 환자는 1.2% 감소, ▴경증 환자는 18.4% 감소한 수치이다. 정부는 비상진료체계가 원활하게 운영될 수 있도록 현장 의견수렴도 지속하고 있다. 5월 2일과 8일에는 「중앙응급의료센터 현장방문」을 통해 현장 근무 직원을 격려했고, 5월 9일에는 「응급의료 관계자 간담회」를 실시하여 응급의료체계 유지를 당부하고 응급의료 관계자의 현장 의견과 애로사항을 청취했다. 또한 같은 날 「상급종합병원-종합병원 간 진료협력 관련 회의」를 개최하여 진료협력체계 운영상 애로사항 및 건의사항도 청취한 바 있다. 앞으로도 정부는 비상진료체계의 원활한 운영을 위해 현장 상황을 지속 모니터링하고, 현장 의료진·전문가 등과 지속 소통해나갈 예정이다. ➋ 의대증원 추진 관련 중수본에서는 의대증원 추진상황을 점검하며, 2천명 증원은 과학적 근거를 가지고 오랜 시간 논의 끝에 정부에서 내린 정책적 결정이라는 점을 강조했다. 최근 일각에서 2천명이라는 숫자가 2월 6일 처음 공개되어 의견수렴이 제대로 이루어지지 않았다는 문제제기에 대해서는 “2035년까지 1만명 부족하다는 추계 결과는 2023년 6월 「의사인력 수급추계 전문가 포럼」 등에서 충분히 논의되어 왔다. 의사 배출에 6년이 걸리는 점을 고려하면 2031년부터 2천명 의사가 배출되어야 부족분을 채울 수 있고, 이를 위해서는 2025년부터 최소 2천명 의대 정원이 불가피하다는 점은 충분히 예상할 수 있었다”고 설명했다. 정부는 2023년 1월 대통령 연두업무보고에서 필수의료 문제 해결을 위한 종합대책 및 의대정원 증원계획을 보고했고, 이후 2023년 10월 ▴「필수의료 혁신전략」 ▴「의사인력 확충 추진계획」, 2024년 2월 ▴의료개혁 4대 과제 추진방향 등을 발표하며 의대정원 증원 추진방안을 구체화해나갔다. 정부는 그간 각계 각층과 다양한 방식으로 130차례 이상 소통하며 의견을 수렴해왔다. 정부는 대한의사협회와도 의료현안협의체를 통해 총 28차례 의견을 나누었다. 특히 28차례 의료현안협의체 논의 중 19차례는 의사인력 수급·배치 등 의사인력 확충 관련 논의를 진행했다. 이 과정에서 공청회 등을 개최하며, 2035년까지 의사 1만명이 부족하다는 증원의 근거를 제시하고 설명해 왔다. 2035년에 의사 1만명 부족분을 채우기 위해서는 2031년부터 2천명 이상 의사 배출이 필요하며, 의사 배출에 6년이 소요되는 점을 감안할 때, 2025년부터 최소 2천명을 늘려야 한다. 정부에서 2035년까지 의사 1만명이 부족하다는 추계를 제시하면서, 의대증원 논의가 본격화 된 작년 하반기부터, 사회 각 계에서는 3천명, 6천명 등 큰 규모의 증원이 필요하다는 의견이 제기됐고, 언론에서도 2천명 이상 증원을 정부가 염두에 두고 있다는 기사들이 보도된 바 있어, 큰 규모의 증원이 이루어질 것이라는 것은 예측 가능한 상황이었다. 정부는 의대정원 증원 규모를 결정하기에 앞서 의료계 의견수렴을 위해 24년 1월 대한의사협회 등 6개 의료단체에 구체적인 의대증원 규모를 공문으로 요청했다. 이 과정에서 한 의사단체는 매년 3천명씩 5년간 15,000명의 증원이 필요하다는 입장을 회신했고, 대한의사협회는 증원에 대한 의견을 제시하지 않았다. 또한 2023년 11월 의대정원 증원 수요 조사에서 의과대학 소재 40개 대학은 2025년 기준 최소 2,151명에서 최대 2,847명 증원을 신청했고, 2024년 3월 40개 대학은 현행 정원 대비 3,401명 증원된 의과대학 정원을 신청하기도 했다. 2월 6일 보건의료정책심의원회에서는 19명의 위원이 2천명 증원을 찬성했고, 의사 위원 3명을 포함한 총 4명이 반대했지만, 반대의 경우에도 규모에 대한 이견으로 증원 자체에는 찬성 의견을 제시했다. ▴3천명 증원이 필요하나 2천명을 증원하고, 수급상황에 따라 주기적으로 모니터링하여 조정해가자는 의견, ▴증원이 너무 늦어 최소 2000명 이상 늘어나야 부족한 부분이 해소되고, 교육 질도 예과 기간을 고려할 때 대응할 시간적 여유가 있다는 의견, ▴의대 증원 확대로 글로벌하게 고령화되고 있는 상황에서 미래 산업의 창출 가능성도 열리며, 지역 문제에도 해결의 실마리가 될 것이라는 의견 등 다양한 찬성 의견이 제시됐다. 정부는 과학적·합리적인 근거와 사회적 논의결과, 각 계의 의견을 종합적으로 고려하여, 보건의료정책심의위원회 심의를 거쳐 2월 6일 의대정원 증원 규모를 발표한 것임을 다시 한 번 밝히면서, 앞으로도 국민의 생명과 건강을 최우선으로 하여 의대정원 증원을 차질 없이 추진해나갈 계획이다. 박민수 부본부장은 “집단행동 장기화 상황에서 병원관계자 및 의사·간호사 등 의료진의 헌신과 국민 여러분의 시민의식 덕분에 비상진료체계를 유지하고 있다. 깊은 감사의 말씀을 드린다”라며 “정부는 현장 의료진을 뒷받침해 환자 진료에 차질이 생기지 않도록 필요한 지원을 아끼지 않겠다”라고 밝혔다. 또한 의대증원 관련 법원 제출 자료와 관련해, “정부는 법원에서 참고할 것으로 판단되는 자료를 정해진 절차에 따라 제출했으며, 성실하게 임하겠다”라고 밝혔다. 아울러 회의록 발언자 익명처리 등과 관련하여, “지금도 의사단체에서는 의대 증원 찬성 의견을 낸 인사들을 공격하고 압박하는 행위가 이루어지고 있어, 위원들의 자유로운 의사 표현을 보호하고, 향후에도 관련 위원회에서 자유로운 토론이 보장되도록 하기 위한 최소한의 조치”라고 말하며, “의사단체가 단체 내부의 생각과 다르다는 이유로, 상대방을 압박․공격하는 비민주적 관행은 즉시 중단되어야 한다”라고 밝혔다.
